Help us spread some Carolina cheer this holiday season! The Employee Forum’s Community Service Committee is sponsoring our second annual campus toy drive benefiting the patients and families at UNC Children’s Hospital! Let’s put a smile on the face of these spectacular mini Tar Heels!

Donation Forms/Procedure
UNC Children’s Hospital General Gift In-Kind Donation Form
The above form (and a receipt) will need to be turned in if the donor wishes to receive a tax deductible receipt from UNC. When filling out the form, subtotals should be used (exclude taxes/shipping costs). If the receipt is unavailable, please submit a printout from a website showing the item with the same cost.
